How do the Yankees wind up with a playoff roster where they have no backup centerfielder, and thus have to pull Bernie Williams (who is terrible defensively anyway) out of the DH slot and put him in CF after they hit for Bubba Crosby?

How is it that, down a run in the 8th inning, Mike Scioscia lets Juan Rivera, Steve Finley, and Adam Kennedy all hit against Mariano Rivera, when he has Casey Kotchman on the bench?
And for that matter, why is Kotchman -- the second best hitter on the team -- on the bench, rather than DHing or playing 1B?
